Privacy Policy

I am committed to the protection of your personal health information and privacy, and follow the standards set out in the Personal Health Information Protection Act (PHIPA) with re‐ spect to the collection, use, and disclosure of your personal information. I will only collect information that is helpful and relevant to our work together. This information will be stored in a secure electronic file for 10 years following the end of our work together. Everything we discuss will be kept strictly confidential between you and me and I will not release information to anyone without your consent. This is an ethical requirement for psychotherapists, and a crucial element of a successful therapeutic relationship.

There are, however, four limitations where I am obligated by law to break confidentiality and report to someone: 

  1. If I have reason to believe you are at imminent risk of harm to yourself or someone else

  2. If I have reason to believe a minor under the age of eighteen years old has been harmed, will be harmed, or has witnessed domestic violence, I am required by law to report this to your local child welfare agency

  3. When a judge subpoenas your notes

  4. If sexual contact by a regulated health care professional is reported

To ensure that I am providing the best quality of care, I seek regular consultation with professional colleagues. Please inform me at any time if you would prefer that I do not discuss your situation (anonymously) with my colleagues. All information is shared in the strictest confidence, as any colleague I consult with also adhere to the laws and ethics of maintaining confidentiality.
